1. What do we do when we receive an Income Withholding Notice and a Termination Notice for the same case on the same date?

Please call the local Child Support Agency (CSEA) at 419-213-3010 for specific instruction about this case.

2. If the employee no longer works for us, has never worked for us or is laid off, what should we do?

You may send the Income Withholding Notice back to the CSEA with a note from you stating the situation. If you know of the person’s new or correct employer, please include this information as well.

3. If the employee has multiple child support orders and does not earn enough to cover all the obligations how do we determine how much to apply to each case?

4. How do I complete the National Medical Support Notice?

Please complete page 2 and pages 13 & 14 if health insurance exists and return those 3 pages to the CSEA.

5. How do I get copies of the Notices that were sent to my business, but I may have lost?

Call the CSEA at 419-213-3010 and ask for replacement copies to be sent to you.

6. We received a Non-Compliance letter but are making payments, why did I receive this?

The notice is generated automatically 45 days after an Income Withholding Notice is sent to an employer. It is sent as a reminder that the employer is legally obligated to withhold. Please double check your records to ensure that child support payments are being properly deducted and remitted. All support payments should include the 10 digit case number and either the court order number or the employee’s social security number. They should be sent to the:
